I applied for a National ID but since it has been 3 years ago, I lost the slip. I could not check if I have a printed ready ID from the website since I lost the TRN. Can you check it for me?

In reference to your concern requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
