Hi I would like to ask the procedure in requesting for a replacement of my national ID?

I lost it somewhere in Batangas last month.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
