Pwede po malaman if meron ng PSA record ang 2 month old baby ko? Kumuha po ng  PSA last month wala pang record. Dalawa po yung kinuha kong kopya kaya nasayang lang yung 500 na ginasto ko para sa PSA.

Thank you for your request dated Sep 17, 2022 10:09:16 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Birth Record.

Your FOI request is approved. 

Please be advised that you may request a copy of your document from PSA after six (6) months from the date of registration and regular transmittal of your document by the concerned Local Civil Registry Office/Consular Office.

For an advance copy of the document, you may apply for a manual verification via the walk-in application at the nearest PSA Civil Registry System outlet and present the following:

1. 2 valid IDs
2. Copy of Report of Marriage/Birth/Despatch
3. Transmittal Details from DFA/LCRO
You may authorize a representative to request on your behalf, kindly provide the following upon application/claim of the document:

a. Original valid ID or photocopy of valid ID of the document owner and original and photocopy of the valid ID of the representative

b. Authorization letter / Special Power of Attorney (SPA) duly signed by document owner and indicating the following:

Type of document
Number of requested documents
Name of the document/s; and
Complete details of requested documents

NOTE: Civil Registration System (CRS) outlets do not accept walk-in clients and only clients with prior appointment can be accommodated. To secure an appointment, please proceed to appointment.psa.gov.ph.
