Hi, I would like to request from your good office for a PSA Negative Certification of our second son, Kyrre Laxus Monterroyo Mendoza, borne on January 15, 2023 around 10 o'clock in the evening at Bacolod City Health Office - Birthing Center; as needed for his late birth registration that was required by the City Health Office for us to process.

A digital copy or hard copy it may be is what we need to complete the requirements requested.

You may request a birth certificate, if yielded negative on our database, you will receive a No Record Certificate.

Please be advised that our CRS Outlets DO NOT accept walk-in clients and only clients with prior appointment can be accommodated. To secure an appointment, please proceed to appointment.psa.gov.ph.
