Hello, I am Crizaline Anne San Diego Agoncillo. I studied at Polytechnic University of the Philippines year 2011 under Bachelor of Science in Business Administration Major in Human Resource Development Management. I did not finish college. I need a copy of my grades for the application of student visa overseas.

Thank you for your request dated Dec 24, 2021 07:48:09 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Copy of Grades.

Your FOI request is approved. Kindly visit PUP Website, (www.pup.edu.ph) then click Online Document Request System (ODRS) under Students’ category. Select your Registrar. Create an account then request TOR, pay and submit the requirements. There will be a status monitoring for your requested document. You will be informed if it is ready for pick-up. Upon pick-up, present client’s copy of application form together with authorization letter  and ID, if claimant is immediate family member or Special Power of Attorney (SPA), if claimant is other than immediate family member.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Vice President Pascualito B. Gatan at pbgatan@pup.edu.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
