July 28, 2021 Dear April Marie Bayona, Greetings! Thank you for your request dated Jul 13, 2021 04:39:03 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for Photocopy of Professional License. Response to your request While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. According to the PUP Open University System Registrar's Office, based on records filed in their office, you submitted the following admission requirements: Original Transcript of Records from PUP-Main Original NSO Birth Certificate Certificate of Employment Medical Certificate Filled-up Admission Form However, the photocopy of your professional licensed was not submitted. You may wish to contact the Professional Regulation Commission at www.prc.gov.ph, or resubmit your request addressed to the correct agency on the eFOI portal at www.foi.gov.ph.
