Hi,hello, Good afternoon. I am Analie V. Polinar, I am previous student of Polytechnic University of Philippines at Don Fabian, Quezon City, 1121 Metro Manila, year 2017-2018.  I am request to  get my Transcript of records because Polytechnic of Philippines  not give my transcript record if no request letter of my present university where I am attended. Please help to get my Transcript of record.

Response to Request

Your FOI request is approved. Kindly visit the PUP Online Document Request System (PUP ODRS) https://odrs.pup.edu.ph . Select your Registrar. Create an account then request TOR, pay and submit the requirements. There will be a status monitoring for your requested document. You will be informed if it is ready for pick-up. Upon pick-up, present client’s copy of application form together with authorization letter  and ID, if claimant is immediate family member or Special Power of Attorney (SPA), if claimant is other than immediate family member.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Vice President Pascualito B. Gatan at pbgatan@pup.edu.ph.
