Hi, I would like to request for clarification of the amendment of company name.

Previously, I was told to email crmd_amend_name@sec.gov.ph, and the process is submission of the following:

1. Sec Reg. No. - 
2. Old name - 
3. New Name - 
4 Name of Requesting Party (name of person) - 
5. Reservation Term - 

However, I was being asked for a notarized certificate even without the name reservation form, or without the name being checked. 

My question is, is this the proper procedure? Submission of Notarized Secretary's Certificate before checking  and reservation of name?
