You asked for Affidavit of No Operation.

Response to Request

With regards to your query, kindly be informed that we have no formatted form for Affidavit of No Operation (ANO) since it's an affidavit format with the reason why the company is not operational. 

Further, as stated in the MC No. 3 s 2021 published in our website at sec.gov.ph: https://www.sec.gov.ph/mc-2021/mc-no-03-s-2021/, please be informed that submission of reports is thru Electronic Filing and Submission Tool or eFAST (https://cifss-ost.sec.gov.ph/user/login). All corporations registered with SEC must enroll in the eFAST in order to access and submit reports.  The Filer may choose under Document type: GIS or AFS then Choose ANO or ANHAM under submission Type and file both AFS or GIS with attached ANO or ANHAM.
