This request requires payment of processing fees as stated in SEC MC No. 3 series of 2017 (https://www.sec.gov.ph/mc-2017/mc-no-03-s-2017-consolidated-schedule-of-fees-and-charges/; https://www.sec.gov.ph/forms-and-fees/schedules-of-fees), as follows:

1. Special program fee of PhP1,000.00; and,
2. Computer processing charge of PhP4.00/minute

2. SEC Registration Number;
3. Date Registered; and the
