Good Day!

I’m an incoming 1st year college student applying for 2024 DOST- SEI Undergraduate Scholarship and I’m in the process of completing the needed documents. One of the documents I need to upload is the affidavit of guardianship, I'm still a minor and both my parents are currently not with me so I declared my eldest brother as my guardian. In regards with that, does DOST-SEI have a specific format of the affidavit of guardianship? If there is, I would like to have it.

I hope to get a response from you as soon as possible.

We regret to inform you that your request is not under Executive Order No. 2 s. 2016 on Freedom of Information in the Executive Branch. We were able to determine that your request cannot be counted as a valid FOI request as it is part of the frontline service of the DOST-SEI, hence your request is denied.
