I have a SSS online account but i forgot my userID and password unfortunately. I also forgot my email account that i used in registering in My. SSS.

May i request that my email address be replaced with [maricrisdordas1029@gmail.com] which is my active email as of date.

I've also attached two copies of my valid ID as proof of my identity.
