I have a SSS online account but i forgot my userID and password unfortunately. I also forgot my email account that i used in registering in My. SSS.

May i request that my email address be replaced [jhaneyyybacit18@gmail.com] which is my active email as of date.

Here are my details relevant to this purpose:

Date of birth: October 18, 2000

SSS/CRN number: 04-4216326-2

I've also attached two copies of my valid ID as proof of my identity.
