Hi, 

I would like to file a motion for reconsideration for my SSS EC Claim for 09/22/2022-10/21/2022. My SSS notification form was signed by my doctor with complete information. It stated that due to long term usage of high volume headset I developed tinnitus to both ears, nausea, vomiting. I was diagnosed with Benign Paroxysmal Positional vertigo caused by my left ear. I was recommended for vestibular rehabilitation and transfer of work to avoid using high volume headsets. Medical certificates were provided signed by my doctor.
