Good Day,

I am applying for relevant skilled assessment from Engineers Australia as part of my application for permanent residency. One of the requirements for the assessment is a summary of my contribution to social security fund which is the SSS in the Philippines. 

I would like to request from your good office a copy of the summary of my SSS contributions from January 2000 to June 2018. 

Your utmost assistance would be greatly appreciated.
