Hello, I would like apply for Sickness Claim. I voluntary pay my contributions. I tried applying through MySSS Online and there are three requirements — Medical Certificate, Affidavit of Undertaking (Sickness) and Medical Records. 

May I know what will I put in the Affidavit of Undertaking Form (Sickness) specificaly items 2 and 3 if I voluntarily pay for my contributions?
