This is Jenina M. Calingo, the granddaughter of Fe N. Calingo. I am writing this letter on behalf of my grandmother's request to process her Annual Confirmation via online. She is already 88 years old and can barely walk. I can send files for her signed request letter, her SSS I.D. and her husband's SSS I.D. (my deceased grandfather), Acknowledgement Stub, picture holding her request letter, UMID and ACOP Form if needed.

Response to Request

ACOP has been temporarily suspended. But if you want to submit the documents, you may go to your nearest SSS branch and leave the documents (enclosed in an envelope) in their dropbox. 

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response.  Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
