Hi Mam/Sir,

I would like to request for financial assistance because I was not able to go to work from Jan 03-15, 2022. My wife and son git tested positive last Jan 3, 2022 then me and my daughter got tested postive last Jan. 07, 2022. We were in quarantine until Jan. 15 with mild symtoms.
With this, i would like to request for financial assistance since I still dont have leave in my new place of work thus, I dont have salary for the time I was absent.
We hope for your understanding and positive reaction.
