Good Day 

I have a SSS online account but I forgot my my UserID and Password unfortunately. I also forgot my email account that I used in registering in My.SSS .May I request that my email address be replaced with maryroseantioquia1@gmail.com which is my active email as of date . And I want to request a Static Information , I can use in my requirements in Local Employment.
