Hi. I would like to request assistance and had a form already but haven't sent it to your office due to my schedule at work and avoid crowded people for the meantime. 
I'm fully vaccinated but still need to get booster. I would like to ask for the steps how to comply EC claim benefit for COVID positive. I had already all the papers, certificates and result from doctor nor clinic.

Do I need to submit it to our HR office nor send it directly to your office instead in SSS main Cebu even without signature of my employer? 
I've stop working 14 -18 days at work. Only 14 days were covered by them.
