To whom it may concern,

Good day! I would like to request for the Financial Assistance your organization is offering.
On September 29, 2021, I was tested positive of Covid-19 through Antigen test at our company. I was quarantined for 1 month due to the persistent cough.
Since I am still a probationary employee I didn't receive any benefits from our company (Sick Leave and Cash Assistance) making our family short on budget for post covid treatment and necessary vitamins.

I hope that you grant my request.
