Good day Sir / Ma'am,

I would like to ask po on how to avail EC Sickness Application Program and acquire EC Voucher for Cash Assistance?

My Employer already submitted the SSS Sickness Notification form including other requirements (MC, Quarantine Certificate & RT-PCR form). SSS Olongapo Branch already approved this application but after checking, there is no EC Voucher.

For update, We forgot to submit the EC Sickness Form, that's why Olongapo Branch didn't provide a EC Voucher.
