I have an SSS Online Account with User ID and password. Unfortunately, I forgot my email account and password that I used in registering in my SSS.

With this regard, may I request that my email address be replaced with mirandaalexander372@gmail.com which is my active email address at present.

Here are my personal details relevant to this purpose.

Date of birth : AUGUST 28, 1959

SSS CRN Number : 06-0693823-3

I've also attached three (3) copies of my valid ID's as proof of my identity.

You may visit and register in our uSSSap Tayo CRMS Portal at https://crms.sss.gov.ph/ so our admins can address your concern. Other details and information on SSS Benefits and Programs can be found in the Knowledgebase and FAQs tab.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing.
