Good day!

I am Espicio M. Amoroso, a Senior Citizen and a member of SSS. I would like request my SSS membership data record from your good office as it is necessary for me to register a SSS account online and to process my application for SSS disablity claim. We lost a copy of it and now we have no reference for the registration. 

I am hoping for your kind and immediate assistance because failure to register will delay my application and we cannot afford to undergo the medical exams again if ever it expires.
