I have a SSS online account but i forgot my userID and password unfortunately. I also forgot my email account that i used in registering in My. SSS.

May i request that my email address be replaced with [emmanueldeguzman21@yahoo.com] which is my active email as of date.

Here are my details relevant to this purpose:

Date of birth: June 21, 1974

SSS/CRN number: 3338627181

I've also attached  copy of my valid ID as proof of my identity.
