Hi ! I would like to request a copy of my E-1 Form since the Original got lost in 2021 by typhoon Odette . For Pre Employment Requirements . And Also I cant create an account online because I dont know what Sss number of my past employer and also I dont know the registered number in my E-1 Form . Hope you can help me . Thank you
