I would like to request this documents for clarification and confirming if there is an illegal transactions behind this agency using the account of my Husband for a Loan. 
How come that my husband has a payment posted 140.00php on October 3,2020, after 12 years of his employer store close we have no contribution made to SSS, the last contribution was made on March 2008 from Employer No. 0720366833 Mc Jola Inc and not by Golden Archer Dev. Corp, and mostly we have already fully paid on the said 12,000 Loan granted on June 16 2006. my husband lost last July 25, 2020, this is for final claim.
