Hello Good Day,

On October 18, 2021, Ms. Namnama Abella sent me an email, indicating that I need to submit a copy of my certificate of separation issued and signed by my employer stating my actual separation date.

On OCTOBER 20, 2021 I submitted clear copy of my certificate of separation. But until now, i did not received any feedback or reply whether or not my submitted copy is correct. Its been 3 months from my application of retirement benefit and i need any status.

please find the attached photographs as evidence and the clear scanned copy of my certificate of employment.
