Hi I would like to request for a replacement SSS id of my deceased father under the name of 
Manuel Tancuachua 
birthdate: Feb 24, 1939
with SSS no. 0200203940 (not sure of SSS number)

due to lost/misplaced SSS id of my father Manuel Tancuachua
if possible to pick up the replacement SSS id from your SSS office Masangkay branch in Sta. Cruz, Manila by me.

You may apply for an upgrade of your UMID through your My.SSS account through the SSS website at www.sss.gov.ph.
