Good day,

I have a sss online account but i forgot
my userID and password unfortunately.
I also forgot my email account that i used in registering in my sss.

My i request that my email address be replaced with [loucolinaresc22@gmail.com]
which is my active email as of date

Here are my details relevant to this purpose.

We also attached two copies of my valid id as proof of my identity.
