hi there. 


i would like to request some information regarding replacement of an old sss id card to umid card.

the case is,

lets say that the old sss id card is in me and i want to have a new sss id/umid. 

is it for free? or do i have to accomplish the r-6 form and pay for the misceleneous fee?

Hi Sir. A member may only request for a UMID card replacement if his old SSS ID/UMID card was lost. A P200 replacement fee is required. For more information, please refer to the SSS website or go to the nearest branch.
