Dear SSS,

Ako posi Nestor G. Castigador with SSS# 07-1464997-7 ay humihiling na ipost ang mga binayad sa aking salary loan dated April 14, 2014 amounting to Php 14,000.00. 
Nakikita ko noon sa aking sss online na ang aking kumpanyang KABUHAYAN MULTI PURPOSE COOP ay nagbabayad ng akina loan simula July 31, 2014 hanggang July 31, 2017.At nang ako po ay nagbayad ng natirang loan kasama na po ang interest at penalty dated May 10, 2021 sa halagang php 11,950 with ref.# G14031505220007, ang mga naunang payment ko po ay nawala sa system.
