Hi, I would like to request for my SSS number. I registered online and accomplished the link the contains the information form. However, I was not able to receive the email that confirms my registration including my SSS number. I also tried to access the link to repeat filling-out the information form but it did not work. I also tried to register again but unsuccessful because the website says that my name was already registered. Thank you in advance for your assistance. Here's my name and birthdate:
