Hi,
Hope all is well.

I would like to request a copy or screenshot of my mother's  Employment History - Employee Static Information from SSS. Although this information is available in the SSS Website, unfortunately, I am not certain of my mother's SSS log in since she has already passed away. I may need to have her employment record for government benefits purposes.

I have included my personal information below in order to retrieve the correct information I needed.
