Warm Greetings!

I would like to ask for an assistance to reset the userID and password in My.SSS Online Account for the reason that I forgot the original details required. When I tried to reset the account using the registered email there was also a problem as it says that it is because of my current membership status and no date of coverage indicated my SSS records.

Kindly help me with the verification process to activate the sss account online using the following details:

Date of Birth: 03/15/1999
