I would like to apply for retirement benefit. Hard copy of duly filled application form with photocopies of pertinent documents/papers submitted to Mr. Roberto V. Roldan when he came to Winnipeg,Manitoba ,Canada on Aug. 16, 2021.  Original documents were presented to him.  Stub from application forms, of my wife Mrs. Leonora V. De Guzman  and mine were handed to us by Mr. Roldan.
