Good evening!

I have my SSS account but  I forgot my old email password.

May I request to replaced my new email account (sherwinborita198722@gmail.com) which is my  active email account as of date.

Here are my details relevant to my request:

Date of birth: September 22,1987

SSS #: 3435090899

I've also attached 2 copies of my ID's as proof of my identity.

Your right to request a review
