Hi SSS,

The current forms available in the SSS Website are those that are scanned copy of the forms and hardly usable to fill in data electronically.

In this FOI request, we'd like to request you to disclose the URLs where the forms (such as SSS form R-1A) are available in fillable format such as Microsoft XLS or Fillable PDF form.
