Good day,

i have a SSS account but when i going to register it on SSS online i already forgot the cellphone number that i've been using before.

May i request that my Cellphone Number 
be replaced with 09451352325 which is my active cellphone number.

Here are my details relevant to this purpose:

Name: Medina, Tessie Mae

Date of Birth: May 26, 1991

SSS/CRN number: 3422671946

I've also attached two copies of my valid ID as proof 
of my identity

You may visit and register in our uSSSap Tayo CRMS Portal at https://crms.sss.gov.ph/ so our admins can address your concern. You may also check other details and information on SSS Benefits and Programs in the Knowledgebase and FAQs tabs in the said portal.

Your right to request a review
