Dear Sir/Ma'am,

I am writing to request a copy of my SSS monthly contributions statement for the period of my employment with my previous employers in the Philippines.

I previously downloaded my monthly statement from my online account. But, this statement however does not indicate my previous employer's name.

I would greatly appreciate it if you could provide me with my contribution statement from specific employers.

2004-2010 with Toshiba Information Equipment Inc Phils 
2011-2020 with Philex Mining Corporation
