Good Day!

I humbly like to request for a financial assistance to help for my family's expenses when I was in quarantine for testing positive to Covid-19 last August 22, 2021 through Antigen and August 25,2021 through RTPCR Test. My quarantine days lasted until September 3, 2021.

Due to that, I was not able to work and can't provide for family for two weeks. And another one week due to UTI. 

I lost my job after that.

If swab test result and quarantine certificate is needed, I can provide it to you.

While our aim is to provide information whenever possible, in this instance this Office does not provide the Covid 19 assistance you have requested for.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
