Hi! Ask ko lang po pwede ko pa bang makuha ang sickness benefits ko for covid 19 kahit lagpas 1 month na akong nakalabas sa isolation facility?

I'm a BPO agent, employed onsite at nagpositive sa COVID-19. October 6 ako nagpositive, October 20 nakalabas sa isolation facility. (Writing this right now at December 01,2021.)

If pwede pa pong makuha ano ano ang need na requirements? And what can be an alternative for 2 valid ID's? As of now i only have 1 valid ID which is postal ID.
