To whom it may concern, 

I am writing to your good office to request for your assistance for an EC claim voucher in order to apply for an ECC Cash Assistance. 

My family and I had been tested positive for COVID-19 last September 27, 2021. The cash assistance would be a great help as my mother was hospitalized and we are all still under medication.

Should there be any other details needed, please let me know.
