Good day,

I would like to ask. I follow all the instructions and gave the required documents needed in the online application for death benefits, after I agree to the terms and conditions , click the proceed button, there's no confirmation if May transactions is successful, but instead it appears in the screen saying " NO RESULTS DEFINED FOR ACTIONS SSS.APP.SYSTEM.CONTROLLER.RDDRACTION and result input"

What does It mean? How can we assure that my transactions is successful?

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing.
