Dear sir! 

Good day to you! My name is Mark Anthony Bolongaita Osumo. 47 years old and resident of Paraiso Homes, Tumaga, Zamboanga City. My SSS # is 33-2197037-9. Last October 15, 2021, I have undergone Laparoscopic cholecystectomy at Davao City. On December 9, 2021, I have filed my sickness benefit claim at your office. Its been more than a month and  I have not received any updates. may  I know what is the status of my application?  I hope you can update me soonest as I will be going to manila next week to start my isolation, in prep for my joining a ship this first week of Feb 2022.
