Im Romualdo Banhaon Jr. My SSS  online account at sss.gov.ph has been locked due to many failed attempts. My I request to unlock or reset my account. My SSS details are:

         Birthdate: 12/24/1975
         SSS Number: 33-4017617-0

Attached below is a scanned copy of my valid ID (any of the following: SSS ID,UMID,PhilHealth ID,etc.)

To address your concern, you may visit and register in our uSSSap Tayo CRMS Portal at https://crms.sss.gov.ph.

We also want to take this opportunity to remind you to be vigilant and refrain from displaying personal data on public platforms as this may be used in fraudulent activities.
