Hi, This is Ms. Jariza Macabanti, currently residing at 32 A.I.B Sagada St. Sto. Domingo (Matalahib), Quezon City. I tested positive both on COVID-19 Antigen Swab last Sept. 22, 2021 and  COVID-19 PCR Swab Test last Sept 25, 2021. I recently recovered/cleared last October 5, 2021. I would like to apply for covid cash assistance for recovery patient. Hope to hear from you soonest.
