Hi i would like to request a cash asisstance for having positive in covid19:

Sept4:having slightly fever
Sept6-7: cough nas colds
Sept8: loss of smell
Sept9: slightly loss of taste
Sept10: request for swab rt-pct test
Sept11: came out the reasult is positive
Sept12: the doctor from the clinic called me and ask me to have my 14days home quarantine since i i have already talk to a doctor for teleconsultation prior my rt-pcr test amd just continue the doctors prescription and told me they are the one to report it in my lgu.
Sept24: the last day of my home quarantine
