Good day..

May I request assisstance on retrieving / receiving my Sickness benefit/claim voucher.
Last June 11, 2021 my sickness benefit / claim was credited to my nominated bank through disbursement account enrollment, but I did not recieve my copy of the voucher.
I need the voucher for filling ECC cash assisstance / claim.

Response to your request

We were able to determine that this particular information is already available online and therefore cannot be counted as a valid FOI request. You may retrieve your requested information by accessing your My.SSS account in the SSS website. An email has also been sent to your registered email address.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
