Good day!
I have a Sss account but i forgot my User ID / password and SSS/CRN number unfortunately. that i use to registering my SSS. 

May i request that my Email address will be replaced with [hannieramojal@gmail.com] which is my active email as of date.

Here are my relevant details to this purpose:

Date of birth:december 29.1998

I've also attached one copies of valid ID to proof of my identity.
